.html and .htm are the same, they are HyperText Markup Language files - the basic language for building web pages.
.php is a file written in PHP which is a server side language and is interpreted by the PHP parser running on the server to produce an html file that gets served to your browser.
.shtml is a html file that is included into another html file by the server (commonly known as SSI - Server Side Includes).

Well yes, it's called an 'a record' (alias record I think)

You need your host to set it

so:

http://bookamagician.co.uk

works

as does

http://www.bookamagician.co.uk

/me runs off spamming merrily
